The reliable way to get the distance is to add the legs. Wikipedia puts Ujjain about 57 km from Indore, and Maheshwar about 91 km from Indore on the north bank of the Narmada River. Added together, and allowing for where in each city you start and finish, the road distance comes to approximately 145-150 km. That matches what the cab operators actually drive: Chikucab states 148 km, and our own Narmada ghats guide lists Maheshwar at about 150 km and 3.5 hours via Ujjain, Indore and Dhamnod.

The 110 km figure is the one to ignore; no road between the two towns is that short. The Rome2rio figure of 159.6 km is plausible for a longer routing but is paired with a drive time no cab will match. Plan on 145-150 km each way and the odometer will not surprise you.
The 3-3.5 hour band that operators quote assumes a clean run through Indore. Leaving Ujjain early in the morning is the simplest way to keep the drive near three hours; leaving mid-morning tends to add a rest stop and Indore traffic. For a same-day return, that difference decides whether you see the ghats in daylight.
Nearly every driver takes this route. From Ujjain you run south on NH52 to Indore, skirt the city, then continue south to Dhamnod and turn west for Maheshwar. Devi Ahilya Bai Holkar Airport sits on the Indore leg, which is why this route also works if you are being dropped at or collected from the airport. Wikipedia notes Maheshwar is connected by State Highway 38, which is the final stretch into town.
A second option heads west from Ujjain towards Badnagar and then south, joining the Dhamnod road without entering Indore. It avoids city traffic, but the sources we checked do not publish a distance or drive time for it, so ask your driver whether it is quicker on the day you travel.

The one-way column is the plain arithmetic of 150 km at the published rate. In practice a one-way outstation hire often costs more than that column because the car has to come back empty; some operators charge the return kilometres, others a fixed one-way fare. Ask which basis applies before you agree a one-way figure.
The round-trip column is the number most travellers actually pay. For a group of four in a Maruti Dzire that is about Rs 2,700 for the whole day at the published rate; six people in a Maruti Ertiga share the same Rs 2,700, which is why the Ertiga is the best value in the fleet for a family. A Toyota Innova Crysta at about Rs 5,100 buys more comfort on the Indore highway, and a Force Traveller at about Rs 6,000 moves 14 people for well under Rs 500 a head.

Treat every number in the table as the base and ask three questions when booking: are tolls included, is there a driver allowance for a long day, and is the kilometre count measured from Ujjain or from the garage. An all-inclusive hatchback quote and a Rs 4,000 sedan one way are not the same product: one is a small car with everything bundled, the other a larger car on an unstated basis. A low per-km headline can also grow once tolls and allowance are added. Compare totals for your date, car and itinerary, not headlines.

According to Wikipedia's entry on Maheshwar, the town sits on the north bank of the Narmada River in Khargone district and is known for Ahilya Fort, the riverside ghats and the Maheshwari saree handloom tradition associated with Ahilyabai Holkar.
