The numbers measure different things. Distance-calculator sites that show about 87 km are usually giving the straight-line (aerial) distance between the two towns, which no road follows. Road planners measure a driving route between town centres, so they land a little above 100 km. The district figure of 98 km is for reaching the temple itself. A few pages quote 45 km, which matches no official or mapped source; ignore it.

From Agar the road continues about 35 km to Nalkheda. Nalkheda is one of the tehsils of Agar Malwa district, alongside neighbouring Susner. The district was carved out of Shajapur district in August 2013, so older sources may still list Nalkheda under Shajapur; it is the same place.
The route passes through small towns and villages, so expect speed-breakers, tractors and two-wheelers even on the better stretches. Carry water, and plan breakfast or a tea stop at Agar in either direction. At Nalkheda, vehicles park near the temple complex on the Lakhundar River; ask your driver to confirm the parking arrangement before you walk in, especially on busy days.
Which option suits you depends on group size, luggage and whether you want to be back in Ujjain the same evening.

The bus is the lowest-cost option, but a same-day return by bus leaves very little time at the temple.
Nalkheda has no railway station. The district administration names Ujjain, 98 km away, as the nearest station, so for most travellers the train only gets you as far as Ujjain Junction. From there you continue by road. If you are arriving in Ujjain by train, the simplest plan is to go straight from the station to Nalkheda by cab.
